Job Responsibilities
- Assess, plan, deliver and evaluate individualised nursing care for patients with acute needs in their own homes
- Work autonomously to recognise deterioration and escalate appropriately
- Administer medicines and treatments while supporting admission avoidance and early discharge
- Collaborate with the multidisciplinary team, primary care and social care partners
- Involve patients and families in care planning and education
- Maintain accurate records and contribute to quality improvement
Requirements
- Essential: Registered Nurse (Adult) with NMC Registration, ability to travel independently, experience caring for acutely unwell adults
- Full UK driving licence and access to own vehicle required for regular travel between sites
- Desirable: community experience, frailty or older persons experience, virtual ward experience
